Ohio Licensed Care Manager RN

A company is looking for a Care Manager, Registered Nurse.

Key Responsibilities

Engage with members telephonically to coordinate their care and develop individualized care plans

Provide disease management education and connect members with resources to support their health goals

Document care management activities accurately and collaborate with the interdisciplinary care team

Required Qualifications

Active and unrestricted Registered Nurse (RN) licensure in the state of OH or compact licensure

Proficient in Microsoft Office Suite and capable of utilizing healthcare technology tools

Minimum of 3+ years of nursing experience

Minimum of 2+ years of case management or home healthcare coordination experience

Access to a private space for remote work and ability to work independently
