Clinic Nurse- RN

Position Purpose:

The purpose of the Clinic Nurse-RN position is to deliver high-quality, patient-centered care by assessing, planning, implementing, and evaluating nursing interventions in collaboration with interdisciplinary healthcare teams. The Clinic Nurse plays a critical role in promoting health, managing chronic and acute conditions, ensuring patient safety, and enhancing overall patient outcomes through evidence-based practices, compassionate care, and adherence to organizational and regulatory standards. As a Clinic Nurse at JWCH, you will have the opportunity to help improve the lives and health status of the underserved segments of the population of the greater Los Angeles area through the direct provision of coordinated health care, health education, health services, and research.

Principal Responsibilities:

1. Collaborates as a core member of the interdisciplinary team to deliver comprehensive, patient-centered medical care.
2. Conducts triage for walk-in patients, performs thorough assessments, creates care plans, and implements evidence-based interventions.
3. Administers oral, IM, SC, and SL medications, as well as treatments and procedures, in alignment with established protocols and standing orders.
4. Performs I.V. placement, monitoring, and fluid infusion as ordered.
5. Assists medical providers with procedures and supports patient care throughout the visit.
6. Monitors and manages residents' physical and mental health needs with sensitivity to behavioral and substance use challenges, following the Harm Reduction Model.
7. Engages in health education and preventive care initiatives to empower residents.
8. Provides culturally sensitive, compassionate support, and addresses patient complaints with appropriate follow-up and resolution.
9. Schedules and coordinates hospital or specialist appointments as needed.
10. Maintains accurate documentation of all patient interactions, care provided, and medication/supply inventory.
11. Completes and documents TB screenings and readings.
12. Prepares, dispenses, and labels medications as ordered; manages medication inventory, removes expired stock, and ensures proper disposal.
13. Maintains and documents the emergency medication kit (E-kit).
14. Places orders for medical supplies and ensures exam rooms are clean, stocked, and compliant with standards.
15. Completes and reviews logs (e.g., refrigerator, oxygen) for accuracy and regulatory compliance.
16. Monitors and completes HEDIS measures, including follow-up on pending items to ensure closure.
17. Accurately and promptly closes patient encounters in the EHR system.
18. Collaborates with the Clinic Administrator to improve clinic operations and workflows.
19. Develops and maintains effective communication among the Medical Director, providers, social workers, back office staff, and external partners to ensure continuity of care.
20. Trains, mentors, and motivates back office staff by identifying educational needs and supporting professional development.
21. Performs other duties as assigned.

Requirements:

1. Possess an active and unrestricted Registered Nurse (RN) license in the state of California.
2. Current CPR card by American Heart Association
3. Minimum of 1-2 years of clinical nursing experience; at least one-year experience in an ambulatory care setting.
4. Proficiency in electronic health record (EHR) systems and familiarity with medical equipment.
5. Excellent verbal and written communication skills to effectively interact with patients, families, and healthcare teams.
6. Experience working with patients and staff from diverse socio-economic, ethnic, and cultural backgrounds.
7. Possess a valid California Driver's license and able to travel to different sites.
8. Bilingual (Spanish/English) preferred.
9. Problem-Solving Abilities: Critical thinking and decision-making skills for assessing and addressing patient needs.
10. Physical Stamina: Ability to stand, walk, and lift patients or medical equipment as necessary.
