[job_card.job_description]Conduct thorough in-home health risk assessments to identify active and chronic conditions, as well as physical, mental, and social needs during visits. Take patient history, perform examinations, and determine diagnoses. Document patient visits in accordance with NCQA standards. Record patient vital signs, when necessary. Place case management referrals and communicate with the Primary Care Provider (PCP) as needed. Engage with patients, caregivers, agency nurses, and other providers to ensure accurate diagnosis. Execute all clinical duties while adhering to OSHA Universal Precautions. Uphold patient confidentiality. Attend required meetings and in-services; participate in committees as requested. Engage in professional development activities, while maintaining licenses and affiliations. Teammates Physicians Medical Staff Patients Caregivers Agency Nurses Providers Vendors Active / unrestricted nurse practitioner license in relevant states. Board certification from ANCC, AANP, or NCCPA. Current CPR certification. Enrollment in Medicare / Medicaid. Valid driver's license with a good driving record. Proficient EHR skills. Experience in geriatric care. Ability to work effectively within a team and maintain positive relationships with patients, staff, and the public. Ability to adapt to various weather conditions and different residential environments. Full range of motion including handling / lifting patients, manual dexterity, eye-hand coordination, normal visual acuity, and normal hearing required. Standing, bending, walking, and stair climbing may be necessary. Regular lifting / carrying of items weighing up to 50 pounds. Ability to travel up to 150 miles daily in urban and / or rural settings, including driving when necessary.
