Responsibilities
The Nurse Practitioner will deliver comprehensive health assessments in residential settings or through telehealth, following the scope of practice as directed by the Collaborative Physician.
Key Duties and Responsibilities
Conduct thorough in-home health risk assessments to identify active and chronic conditions, as well as physical, mental, and social needs during visits.Take patient history, perform examinations, and determine diagnoses.Document patient visits in accordance with NCQA standards.Record patient vital signs, when necessary.Place case management referrals and communicate with the Primary Care Provider (PCP) as needed.Engage with patients, caregivers, agency nurses, and other providers to ensure accurate diagnosis.Execute all clinical duties while adhering to OSHA Universal Precautions.Uphold patient confidentiality.Attend required meetings and in-services; participate in committees as requested.Engage in professional development activities, while maintaining licenses and affiliations.You will collaborate with. . .

Required Knowledge, Skills, and Experience
Active / unrestricted nurse practitioner license in relevant states.Board certification from ANCC, AANP, or NCCPA.Current CPR certification.Enrollment in Medicare / Medicaid.Valid driver's license with a good driving record.Proficient EHR skills.Preferred Knowledge, Skills, and Experience

Ability to adapt to various weather conditions and different residential environments.Full range of motion including handling / lifting patients, manual dexterity, eye-hand coordination, normal visual acuity, and normal hearing required.
