  • \n \n\nJob Description\n

    The Resident Nurse Care Manager (RNCM) is responsible for establishing a comprehensive care practice that entails patient assessments, care planning, implementation, coordination, and ongoing evaluation to ensure both medically and financially appropriate care is provided. Key responsibilities include :

    \n \n

  • Facilitating interdisciplinary team care coordination.
  • \n

  • Managing the care of participants recently discharged from acute care centers and skilled nursing facilities.
  • \n

  • Collaborating closely with the interdisciplinary team, including Social Workers and Physicians, to collect essential medical and social information.
  • \n

  • Conducting assessments at participant residences when necessary, with mileage reimbursement provided for travel.
  • \n \n

    This position is primarily on-site, with occasional community visits required.

    \n\nProgram Overview\n

    The role is part of a Program of All-Inclusive Care for the Elderly (PACE) focused on keeping older adults as healthy as possible while living in the community through clinics, home visits, and additional services.

    \n\nRequirements\n \n

  • Must be open to transitioning to a permanent position after the initial contract period.
  • \n

  • Experience with the geriatric population is highly preferred.
  • \n

  • First-time candidates are welcome to apply.
  • \n

  • Weekend availability is not required.
  • \n

  • No on-call duties are expected.
  • \n

  • Floating between units is not required.
  • \n

  • Local candidates are welcome, but relocation is not mandatory.
  • \n

  • Pending state license accepted.
