[job_card.job_description]Years of Experience : 1 year(s) Travel Experience Required : No Certifications Preferred : BLS Case Management / Utilization Review Admission Criteria Care Coordination Discharge Planning Utilization of InterQual Criteria Utilization of Milliman Guidelines Utilization Review Regulatory Knowledge (CMS : Centers for Medicare and Medicaid Services) Pre-Approved Time Off : None Patient Age Groups : Geriatrics Interdisciplinary Support : Phlebotomy Special Procedures : Experience with either Milliman Guidelines or InterQual Criteria required Facilitating interdisciplinary team care coordination. Managing the care of participants recently discharged from acute care centers and skilled nursing facilities. Collaborating closely with the interdisciplinary team, including Social Workers and Physicians, to collect essential medical and social information. Conducting assessments at participant residences when necessary, with mileage reimbursement provided for travel. Must be open to transitioning to a permanent position after the initial contract period. Experience with the geriatric population is highly preferred. First-time candidates are welcome to apply. Weekend availability is not required. No on-call duties are expected. Floating between units is not required. Local candidates are welcome, but relocation is not mandatory.
