[job_card.job_description]F / T Day Care Manager MSW at our Little Company of Mary Hospital in Torrance, CA. Positions available are full-time and will work 8-hour Day shifts. Master's Degree Social Work. 1 year 1-3 years recent experience in acute care inpatient setting with patient population in area of service. 6 months Advanced communication skills to communicate effectively with patients, hospital staff, physicians and the community. Computer experience / literacy as needed for the job. 6 months Knowledgeable of community resources with care coordination and discharge planning experience preferred. Seniority Level - Associate Minimum Education - High School Diploma or Equivalent Willingness to Travel - Occasionally Do you hold a Master's Degree in Social Work? Do you have 2 years of recent experience working as a Social Worker in an acute care / hospital setting? Do you have 2 years of recent experience working as a MSW in Care Management of a hospital? Do you have 1 year of recent experience placing behavioral health patients in an acute care setting? Do you have 2 years of recent experience working directly in Hospital Discharge Planning and Care Coordination
