About the Role :

As an Occupational Therapist, you’ll provide personalized therapy in a home setting, helping patients regain independence in daily living activities through customized treatment plans and adaptive strategies.

Key Responsibilities :

Conduct comprehensive patient assessments and establish realistic therapy goals.

Develop personalized treatment plans focused on ADLs, motor skills, cognitive function, and environmental modifications.

Provide hands-on occupational therapy services, helping patients increase mobility and functionality.

Educate patients and caregivers on adaptive techniques, energy conservation, and safety strategies.

Collaborate with nurses, physical therapists, and other healthcare professionals to provide comprehensive care.

Maintain accurate and timely documentation following regulatory standards.

Stay current on home health regulations and evidence-based therapy techniques.

Qualifications :

Valid State License as an Occupational Therapist

Minimum 1 Year of Experience in home health or acute care

Strong clinical skills with a patient-first mindset

CPR Certification (Required)

Excellent communication & interpersonal abilities

Reliable transportation for home visits

Understanding of Medicare home health regulations & documentation
