Nurse Practitioner - TX, LA, TN Licensed

A company is looking for a Nurse Practitioner for its National After-Hours Team, licensed in TX, LA, and TN, on a part-time basis.

Key Responsibilities

Provide telephonic support and medical care for residents and facility staff after-hours

Coordinate care between facilities, hospitals, and clinics using a telephonic platform

Deliver comprehensive assessments and document encounters in the EMR while ensuring compliance with CMS requirements

Required Qualifications

Master's Degree as a Nurse Practitioner

Active, unrestricted licensure in Texas and Louisiana, or Tennessee

Nurse Practitioner national certification as ANP, FNP, or GNP

3+ years of experience as a Nurse Practitioner

Background in acute and chronic disease management
