[job_card.job_description]Provide, coordinate, and direct the provision of home nursing care according to physicians' orders based on agency policies and procedures through the competent application of the nursing process. Responsible to the Clinical Manager and directly supervise and evaluate Home Health Aides and Licensed Practical Nurses / Licensed Vocational Nurses. Conduct home visits as required, with periodic office meetings. Cover admissions and other needs as they arise, with flexibility in travel distances and work hours. Manage a daily average of 5-8 patients, equating to 6-8 points, with a weekly productivity expectation of 30 units. Types of cases include Start of Care, Wound Care, Follow-up Visits, etc. Participate in on-call rotations as needed with one weekend per month, with the potential for additional optional on-call hours. Complete all documentation within 24 hours of the patient appointment to ensure timely billing and patient care follow-up. Open to travel up to a 75-mile radius from the home clinic and adaptable to potentially being floated to nearby clinics to meet patient needs. Candidates must be comfortable with driving to patients' homes daily and are expected to accept every patient on their schedule. Open to local applicants at the same rate. There are no guaranteed hours with this contract, and clinicians must meet weekly productivity points.
