We are seeking a qualified Registered Nurse RN interested in joining our Case Management team at our Claymont, Delaware location.

The Registered Nurse RN provides the coordination of the behavioral and physical health care needs of the program participants, monitors participant’s medication, and provides psychiatric nursing services consistent with physician’s orders. Some of the responsibilities would include the following :

• Monitor medication, documenting participants compliance, medication efficacy, and side effects as reported. Supervises participants in self-administration of medication as needed.
• Assist in the development, implementation, evaluation and revision of recovery plans in collaboration with the participant and assigned staff.
• Act as a liaison and consult with community agencies and families to maintain coordination of care in the treatment process.
• Provide ongoing assessment of participant’s mental health symptoms and responses to recovery plans ensuring appropriate and early intervention.
• Ensure annual review of the participant’s physical condition and encourages compliance with treatment for identified physical problems.
• Provide on-call crisis intervention coverage as assigned.
