Job Details

  • Physical Therapist (PT) position at Trinity Health At Home – Muskegon in Michigan.
  • Role involves working Monday through Friday with no weekends or holidays, includes holiday rotation.
  • Use of EPIC system is required.

    • Job Requirements

  • Must have valid state licensure with verification dated within 30 days of start.
  • American Heart Association BLS certification required for all direct patient care personnel.
  • Completion of Trinity Specialty Specific Competency Testing within a year.
  • Proof of COVID-19 vaccination or approved exemption required.
  • Proof of MMR vaccination or immunity required.
  • Fit test for mask (Iowa - 1860 mask only) required.
  • Hepatitis B vaccination series required prior to start; titers not accepted.
  • Influenza vaccine or approved exemption required during flu season.
  • Valid Driver's License / State ID required.
  • 2 Step PPD or IGRA test required; positive TB requires chest x-ray within last year for Michigan.
  • Proof of Varicella vaccination or immunity required.
  • Physical / Health Statement documenting fitness to work within last year required.
  • Trinity 10 Panel Drug Screen completed within 30 days of start required (no THC).
  • Completion of various Trinity professional and background checks including criminal background, education verification, EPLS, fingerprinting, GSA / OIG, integrity and audit services form, joint commission testing, NSO, system access form, code of conduct form, and I9 form.
  • Trinity Skills Checklist provided by agency required for clinical contractors.

    • Schedule Information

  • Monday through Friday schedule.
  • Holiday rotation included, no weekends or holidays.
