Job Summary :
As a Registered Nurse, you'll bring expertise, confidence, and decisiveness to every interaction. You'll provide direct and indirect nursing care to participants, managing complex clinical situations and ensuring care is delivered safely and effectively.

Schedule :
First shift, full-time hours. Approximately 20% of your work will involve home visits. Specific work hours will be established in consultation with your supervisor.

Duties / Responsibilities :
Provide direct and indirect nursing care to participants following the Care Plan.Collect and analyze patient data, set priorities, and identify problems needing follow-up.Develop, implement, and evaluate plans of care, collaborating with interdisciplinary teams.Provide education, guidance, and discharge instructions to participants and families.Respond independently to emergencies and implement advanced care as needed.Document care, observations, and participant outcomes accurately in the electronic medical record.Communicate participant changes and updates clearly to the care team.Participate in Quality Improvement initiatives and clinical process enhancements.Travel to participant homes or other sites as needed to provide care.Everyone's journey is unique.
