Physical Therapist Assistant (PTA) Per-Diem

Linwood Meadows Care Center, a premier skilled nursing facility in Visalia, CA, is hiring a Per-Diem Physical Therapist Assistant (PTA) to join our dedicated rehabilitation team. If you're a licensed PTA who is passionate about patient-centered care and values working in a supportive, collaborative environmentthis is the place for you!

Position Summary : The Physical Therapist Assistant provides physical therapy treatments under the direction and supervision of a licensed Physical Therapist. This role supports residents in achieving their mobility and recovery goals as part of an interdisciplinary care team.

Key Responsibilities

  • Deliver therapy services in accordance with the treatment plan established by the Physical Therapist
  • Monitor patient progress and communicate findings to the supervising therapist
  • Accurately document daily treatments, patient responses, and progress using systems such as Casamba and PointClickCare
  • Assist with discharge planning and family education regarding home exercises or mobility assistance
  • Ensure compliance with all federal, state, and facility guidelines and policies
  • Collaborate with nursing, therapy, and administrative teams to support quality resident care

Qualifications

  • Associate's degree from an accredited PTA program
  • Current PTA license in good standing in the State of California
  • Skilled nursing or post-acute care experience preferred
  • Strong communication, teamwork, and organizational skills
  • Proficient with electronic medical records and therapy documentation platforms
