Community Care is excited to invite a passionate Physical Therapist to become a vital part of our dynamic team. In this full-time role, you will work in our state-of-the-art facility, Wellness Way , focusing on outpatient care.

Your Responsibilities :

As a Physical Therapist, you will deliver direct patient care while integrating patient / client management strategies. Your focus will be on maximizing functional outcomes through detailed evaluation, diagnosis, and personalized interventions. You will also be responsible for accurately documenting treatments and patient progress in accordance with professional standards.

Key Responsibilities :

  • Develop comprehensive treatment programs based on patient evaluations, including medical history and functional status.
  • Prioritize patient care needs effectively.
  • Evaluate patient needs and provide timely recommendations for treatment, equipment, and education.
  • Maintain thorough and accurate documentation per organizational compliance standards.
  • Ensure clinical records meet all regulatory requirements and are clearly presented.

    • What You Will Need :

  • A degree from an accredited Physical Therapy program (Bachelor, Master's, or Clinical Doctorate).
  • Valid NYS Physical Therapy license and registration.
  • Preferred : 1 year of physical therapy experience, but new graduates are encouraged to apply!
  • Excellent verbal and written communication skills.

    Compensation : Annual salary ranging from $78,000.00 to $118,577.28.
