Novice LPN - Private Duty

RYZ Labs is hiring a Novice LPN - Private Duty to provide services to patients / clients in their places of residence.

Responsibilities :

• Utilizes the nursing process to plan, implement, and evaluate patient / client care and / or contracted services
• Demonstrates on each assignment the provision of skilled nursing care rendered in accordance with prescribed orders
• Identifies and documents changes in the patient's both physiologic and psychosocial condition
• Notifies Director / Supervisor if applicable on the following;
• Physician notification to obtain necessary orders for intervention(s) per state regulations
• Changes in patient / client condition
• Adheres to Maxim's policies and procedures and applicable protocols
• Identifies and prioritizes actions based upon patient / client care requirements
• Assures patient safety at all times and is the recipient of services provided
• Verifies that documentation reflects patient / client collaboration, participation in plan of care, expected patient / client outcomes, and selected nursing interventions
• Performs interventions, treatments, and medication administration, according to identified priorities, plan of care, facility policies, and patient / client care outcome standards as applicable.
• Monitors and documents the effectiveness of interventions, treatments, and medications
• Evaluates patient / client and provides a safe, comfortable, and therapeutic patient / client environment
• Monitors the safety and effectiveness of the environment and equipment, including universal precautions
• Participates in activities to facilitate good interpersonal communications
• Communicates observations and significant changes in patient / client status to the Director of Clinical - - Services and / or Clinical Supervisor / Case Manager or licensed designee
• Documents all physician collaboration / coordination of care in the medical record and

communicates this information as needed
