Hours : Full Time

Shift : Day Shift

Location : Primary Care - Wareham, MA

• Driving on average 3 or more times per week required

Under the direction of the Director of Integrated Behavioral Health Operations performs a wide variety of duties in Southcoast Physician Group (SPG) Practices. Provides direct clinical service for an array of general behavioral health conditions in an integrated team-based model of care Clinical service may be pre-scheduled and / or urgent basis and include consultation / assessment, support and linkage to services, psychoeducation, brief interventions, and counseling to promote positive health outcomes. Participates in quality improvement initiatives and assists in developing behavioral health programming as requested.

Compensation : Pay rate will be determined based on level of experience.
