[job_card.job_description]Masters degree in Social Work (MSW) required. Minimum 2 years of experience working with the gerontology population. Experience in behavioral health health plans case management counseling home health or hospice. Valid drivers license reliable transportation and auto insurance (for occasional travel). Ability to work independently exercise sound judgment and communicate effectively. Proficiency in Microsoft Office (Word Excel Outlook PowerPoint) and related documentation systems. Bilingual in English and one of the following : Arabic Farsi Chinese Korean Russian Spanish or Vietnamese. Prior experience working in managed care PACE or community-based programs. Strong interpersonal analytical and organizational skills. Ability to manage multiple priorities in a fast-paced remote environment. Experience building rapport and maintaining effective working relationships with multidisciplinary teams. Excellent written and verbal communication skills. Full Telework / Remote Role Monday Friday 8 : 00 a.m. 5 : 00 p.m. May require occasional travel for training or community outreach.

Aroha Technologies a trusted healthcare staffing partner With CalOptima Health is seeking a Social Worker to join our clients Managed Care Services Program (MSSP) team. This is a fully remote opportunity for a dedicated professional passionate about serving diverse and elderly populations through coordinated patient-cantered care.
Key Responsibilities
Care Management (70%)
- Collaborate with an interdisciplinary care team to address psychosocial needs such as financial concerns transportation and community support services.
- Coordinate member care needs and utilize community resources effectively.
- Provide consultation education and advocacy to members and families.
- Analyze assessments to identify individual needs and available resources.
- Monitor ongoing services and ensure appropriate utilization of support systems.
- Develop and maintain individualized service and care plans.
- Initiate referrals and follow-up for community-based support programs.
- Act as a liaison between members families and healthcare providers to achieve care goals.
- Maintain comprehensive documentation and case management statistics.
- Conduct or participate in case conferences and care coordination meetings.
