BayCare Health Systems is currently seeking a Discharge Planner 1 to join our outstanding and compassionate team at the Behavioral Health Center!

  • Facility : Behavioral Health Center
  • Location : Tampa, FL
  • Status : Full time, Hourly
  • Shift Hours : 8 : 00 am- 4 : 30 pm
  • Weekend Work : Occasional
  • On Call : No

The Discharge Planner I responsibilities include :

  • Provides assessment, linkage, monitoring, planning and advocacy for adult and or children experiencing acute mental health and or substance abuse issues in an acute care inpatient setting.
  • Possesses a clear understanding of discharge and transition planning and linkage with community resources.
  • Oversees legal status issues related to the individuals involuntary or in accordance with the Baker Act regulations.
  • Provides support as active member of the treatment team.
  • Will perform other duties as assigned by supervisor.

    • Travel may be required.

    Required Certification :

  • BLS (Basic Life Support)

    • Required Experience :

  • Behavioral Health - 1 year
  • Child and Adolescent - 2 years (SIPP Only)
  • Acute Care - Preferred 1 year

    • Required Education :

  • Bachelors - Counseling / Health Education / Related Field / Social Work / Human Services / Psychology / Nursing
  • Master's in human services - Preferred
