Pride Health is hiring a Nurse Practitioner in Albany, NY! (Contract)

Work in 2 Long Term Care settings in Albany, NY / Rennsselaer County

Schedule : Monday-Friday (8am-5pm)

Contract : Potential Temporary to Permanent

Pay Rate : $65-70 / hour

Specific Duties :

Provide specialized care to high-risk health plan members
Work as part of an interdisciplinary team
Conduct comprehensive assessments
Manage acute and chronic medical and behavioral conditions
Ensure accurate documentation, including ICD-10 coding
Communicate and collaborate with the care team
Facilitate advanced illness and care planning discussions
Offer counseling and education to patients and caregivers

Must have qualifications / experience :

Nurse Practitioner or Physician Assistant

NP Certified in one of the following (FNP, ANP, GNP)

Access to transportation

1+ year of experience (LTC, SNF, Geriatric preferred)
