Physical Therapist

An exciting opportunity is available for a Full-Time Physical Therapist (PT) to join our team at a Skilled Nursing Facility (SNF) in Monticello, Minnesota . This role is ideal for both new graduates and experienced professionals looking to grow their careers in a supportive, flexible, and dynamic environment. New graduates are welcome to apply!

This position offers a variety of career growth opportunities, including professional mentorship, unlimited Continuing Education Units (CEUs), and the chance to work in a network with over 100 facilities across 11 states .

Key Responsibilities :

  • Patient Assessments & Treatments : Assess and treat patients based on individualized care plans to meet their unique needs.
  • Documentation : Accurately document patient progress, treatments, and outcomes in a timely manner.
  • Program Development : Plan, market, and develop new programs to enhance patient care and improve overall outcomes.
  • Effective Communication : Collaborate with patients, families, physicians, and healthcare professionals to ensure coordinated care.
  • Staff Education : Lead in-service training sessions for fellow therapists and staff.
  • Clinic Operations : Assist with the general operations of the therapy department, maintaining high standards of care and efficiency.

Qualifications :

  • Licensure : Current Physical Therapist (PT) licensure in Minnesota (or eligibility to obtain licensure).
  • New Graduates Welcome : We encourage new graduates to apply!
  • Skills : Strong communication skills with a compassionate, patient-centered approach to care.
  • Teamwork : Ability to work independently as well as part of a collaborative healthcare team.
  • Interest in SNF Setting : A genuine interest in working within a Skilled Nursing Facility (SNF) environment.
