Your Responsibilities :

  • Travel to client homes within assigned service areas to deliver personalized care.
  • Conduct functional and cognitive evaluations to design individualized therapy plans based on Medicaid and state guidelines.
  • Administer restorative and maintenance treatments, overseeing OTA contributions while adhering to billing regulations.
  • Offer expert advice on home modifications, assistive devices, and caregiver training to promote improved functional outcomes.
  • Keep precise records of the services provided and collaborate with care teams to align therapeutic interventions with overall treatment goals.

    • What You Need to Succeed :

  • Current MN Occupational Therapy License.
  • Graduation from an accredited Occupational Therapy program.
  • CPR certification.
  • Access to a reliable vehicle, along with a valid driver's license and insurance.
  • Preferred experience in home care or working with geriatric populations.
