Social Worker

Lincoln Park Healthcare Campus is seeking a Full Time Social Worker position .

QUALIFICATIONS :

1. Graduate of an accredited school in social work or related field; having a bachelor’s degree in social work or a bachelor’s degree in a human service field including but not limited to sociology, special education, rehabilitation counseling, and psychology.

2. State certified by the Board of Social Work Examiners as CSW or LSW.

3. Experience in Subacute and Long Term Care.

POSITION SUMMARY :   Performs social services based on the facility’s policies and procedures as regulated by the State and Federal laws. PERFORMANCE EVALUATION : A competency based performance evaluation has been made part of this job description.

JOB RESPONSIBILITIES

Accurately conducts social service assessments and interventions and documents accordingly within the facility’s standards based on the State, Federal regulations & Joint Commission standards.
