Patient Care Coordinator

A company is looking for a Patient Care Coordinator in the Eastern Time Zone.

Key Responsibilities :

Coordinate and schedule in-home and virtual appointments for providers and clinical staff

Utilize scheduling software to manage schedules efficiently

Collaborate with operational and clinical teams to address scheduling gaps and patient needs

Required Qualifications and Education :

High school diploma or GED

2+ years of work experience in a healthcare setting

Preferred experience in call center or scheduling roles

Preferred understanding of patient communication protocols

Preferred experience in a value-based healthcare setting
