Nurse Practitioner - Geriatric Primary Care (SNF, LTC, ALF)

Job Description

We are seeking an experienced Nurse Practitioner to provide high-quality, patient-centered primary care in Skilled Nursing Facilities, LTC, and ALFs.

• Deliver comprehensive assessments, manage acute and chronic conditions, and develop treatment plans.
• Order diagnostics, prescribe medications, and educate residents, families, and staff.
• Collaborate with care teams to ensure seamless patient support and documentation.
• Enjoy flexible scheduling, autonomy, minimal on-call duties, and a supportive team environment.

Qualifications include : Minnesota NP license, board certification, and preferred experience in geriatric or post-acute care.
