Location : London Kentucky with weekly outreach clinic in Stearns KY and biweekly outreach clinic in Danville.

Job Overview :

We are seeking a dedicated and skilled Nurse Practitioner (NP) to join our vascular team. In this role, you will collaborate closely with vascular surgeons to deliver seamless care plans for patients. Key Responsibilities :

• Provide direct patient care at outreach clinics in multiple locations, assessing and managing vascular conditions.
• Work closely with our vascular surgeons to create and coordinate care plans for specific patients.
• Serve as a liaison between patients, surgeons, and outreach clinics to ensure cohesive communication and care delivery.
• Coordinate follow-up appointments, procedures, and patient education.
• Learn and apply the fundamentals of vascular surgery through provided training.
• Maintain detailed and accurate patient records.
• Collaborate with the healthcare team to improve patient outcomes and care efficiency.
