About the Role :

We are seeking a dedicated and passionate Physical Therapist to join our team at One Health Home Health in Contra Costa. As a PT, you will have the opportunity to provide exceptional care to our patients in the comfort of their own homes, helping them achieve their rehabilitation goals and improve their quality of life.

Responsibilities :

• Conduct patient evaluations and develop individualized treatment plans
• Provide hands-on therapy and exercises to promote mobility, strength, and balance
• Educate patients and their caregivers on at-home exercises and proper techniques
• Collaborate with physicians, nurses, and other healthcare professionals to ensure comprehensive care
• Document patient progress and communicate effectively with the interdisciplinary team

Requirements :

• Active Physical Therapist (PT) license in the state of California
• Minimum of 1 years of clinical experience, preferably in home health or geriatric care
• Strong interpersonal and communication skills
• Ability to work independently and manage a flexible schedule
• Passion for providing compassionate and patient-centered care
