  • Travel Pay Rate : (above 50 miles)

  • Hourly : $24 / hr W2 + $1,200 / week (non - taxable stipend)
  • Gross : $2,160 / weekly

    Position Summary :

    The LPN Case Manager supports patient care through coordination, documentation, communication, and follow-up activities. This role requires strong organizational skills, clinical knowledge, and the ability to advocate for patient needs across multiple healthcare settings.

    Roles & Responsibilities

  • Assist in the development and execution of comprehensive care plans under RN or physician supervision.
  • Conduct patient assessments, gather clinical data, and identify care needs or service gaps.
  • Coordinate with physicians, RNs, social workers, and external providers to ensure continuity of care.
  • Monitor patient progress, update care plans, and document all interventions in the EMR system.
  • Communicate with patients and families regarding treatment plans, follow-up appointments, and support resources.
  • Facilitate referrals for home health, therapy services, DME, or community programs.
  • Perform medication reconciliation, verify orders, and support patient education.
  • Ensure compliance with regulatory requirements, quality standards, and facility policies.
  • Advocate for patient needs, ensuring timely and appropriate service delivery.
  • Assist with discharge planning and transition-of-care processes.
  • Required Qualifications :

  • Active LPN license of CA
  • BLS
  • Minimum 2 years of recent experience.
  • EPIC experience.
