Your Role and Responsibilities :
- Conduct thorough adult / geriatric assessments, including reviewing medical history, diagnosis, treatment plans, and more.
- Administer and order necessary lab tests to provide optimal patient care.
- Collaborate effectively with physicians, case managers, and family members to create a comprehensive care experience.
- Engage directly with patients in their homes or Skilled Nursing Facilities, delivering care tailored to their unique needs.

Master's Degree or equivalent experience and completion of NP licensure.Current NP licensure in your practice state, with prescription authority or the ability to obtain it.AANP, AACN, or ANCC certification in an applicable specialty.Current BLS, ACLS, or CPR certification.1 year of NP experience preferred, though new graduates are strongly encouraged to apply.A valid state driver's license and proof of adequate automobile insurance.Strong computer skills and proficiency with health / medical record software.Excellent verbal and written communication skills.Our Culture at Matrix :
