Lung Nodule Coordinator
Responsible for coordinating the individualized care of patients throughout the continuum of care services, from abnormal findings of a lung nodule to end of treatment, in order to increase and assure timely access to healthcare services. Will be responsible for helping manage patients who have incidental lung nodules, as well as nodules found from lung cancer screening exams. Will be an instrumental part in helping grow the program for the system.
Serves as a member of the multidisciplinary care team between thoracic surgery, pulmonary, and radiology, functioning as a patient advocate, educator, key point of contact and liaison between the patient and all other appropriate services.
Manages referrals, schedules consultations, order and schedule appropriate testing in a timely and efficient manner. Follows up on diagnostic testing.
Navigates patients through the continuum to eliminate barriers to health care and enhancing patient and physician satisfaction.
Functions as a collaborating member of the health care team, but also must be able to act independently to assess patient and provider needs to ensure appropriate handoffs so that patients receive services promptly and efficiently.
Applies the nursing process as appropriate within the context of the organization's framework for professional practice and in accordance with guidelines established by the team.
Participate as needed in community education outreach programs / workshops / seminars / support services.
Provide coverage as necessary for thoracic surgery nursing responsibilities that may include triaging patient calls, pre-operative instructions, and consultation management.
Utilizes EPIC for tracking data to help improve the program and increase efficiency in following patients.
Responsible for weekly MDC meeting which includes moderating meeting, managing agenda, managing CME, taking minutes and attendance, and communication with multidisciplinary team.
Qualifications
Licensed as a professional RN in the state of Connecticut
BSN preferred
Minimum of 5 years nursing experience
EPIC experience
