Primary Care Family Nurse Practitioner opening in Lancaster, CA

Job Description

Seeking FNP to join talented team in Lancaster, CA.

This position will play vital role in the Health Center's Primary Care program. The successful candidate will be an advanced practice registered nurse who has acquired the necessary training to provide primary healthcare services to patients. The candidate will be responsible for caring for patients, maintaining accurate and current patient records and scheduling and administering follow-up appointments to patients as needed. The candidate will also be able to develop an appropriate treatment plan and provide effective management of patient care with proper evaluation. The candidate can also function as an educator to families, staff, and the community.

• This is a Full-time position. Days and hours of work are Monday through Saturday, hours range from 7:30am-6:30pm with occasional approved overtime.

Skills / Experience

• 1+ years experience
• A keen interest in helping the underserved population (e.g. the homeless population).
• FQHC Experience a plus
• Familiarity with Quality Measures a plus
• Bilingual (English and Spanish) a plus

• Medically Assisted Treatment(MAT) integrated with Primary Care
• Individual Counseling
• Drug and Alcohol Dependency (Non court-ordered)
• Alanon Family Group (Tuesdays at 5:15-6:15PM, open to the community)
• SMART recovery group (Open to BACHC Primary Care patients, call for more information)
• Plus other special groups for specific issues

Compensation & Benefits:

• $90-130K per year
• Health, vision, and dental insurance, 403B (up to 6% match), PTO and sick time, CME, tuition reimbursement
