Open Door Family Medical Center provides health care and wellness programs to individuals and families in need throughout Westchester, Putnam and Ulster Counties. A pioneer Federally Qualified Health Center, Open Door provides more than 300,000 patient visits annually and serves nearly 60,000 individual patients who might not otherwise have access. Primary medical care, dental care, integrated behavioral health care, clinical nutrition, wellness programs, and chronic disease management are the foundation of its clinical programs.
RESPONSIBILITIES :
- Identify, screen, and triage patients for intervention daily
- Provide brief intervention treatment to patient and / or family
- Conduct screenings using appropriate screening tools to identify school readiness and monitor developmental milestones
- Document symptoms, problems, screening results, and interventions provided
- Develop treatment plan, goals, and objectives with patient
- Provide and facilitate communication with treatment care team providers, both medical and behavioral health, and within the community
- Incorporate health and wellness education and referrals for preventive care
- Respond to and incorporate physical health concerns into brief treatment interventions
- Provide crisis intervention and Warm Hand Off assessments to behavioral health staff
- Participate in weekly group and individual supervision, and clinical training seminars
REQUIREMENTS :
Masters in Social Work, in Marriage and Family Therapy, or in Mental Health CounselingCurrent New York State registration and licenseBilingual in English and SpanishRecent clinical experience or trainingExcellent clinical knowledge with mentally ill and substance abusing populationKnowledge of evidenced based treatment modalities and crisis managementExcellent written and verbal communication skillsExperience in working with lower socioeconomic and multicultural populationsAbility to work collaboratively with other staff, treatment providers, and community agenciesHighly organized, self directed, able to multi-task in a fast-paced environmentStrong computer skills including working knowledge in Word and Excel, as well as capacity to navigate and enhance proficiency in these programsPatient centric with empathy, excellent active listening and communication skills, engaging and efficientPassionate about providing high quality care to low income, under-served residents and those lacking access to health careThere are many fantastic benefits to a career at Open Door Family Medical Centers.
