The Hospital Liaison Outreach & Engagement Specialist JOB PURPOSE / SUMMARY Summary of role of team : The HLOEP Program, by virtue of the persons served, represents a place where the Center's vision is lived out on a day-to-day basis. The severe and / or persistently mentally ill (SPMI) population truly embodies the poorest of the poor and the sickest of the sick. Therefore, it is the philosophy of the HLEOP Program to live up to this vision by reaching out to the SPMI regardless of what situation they are in or where they are and by opening a door of stability through mental health treatment. The Hospital Liaison and Outreach Engagement Specialist requires the staff member and consumer / natural support to have a strong, productive relationship, which includes viewing the consumer / natural support as a responsible partner in identifying and obtaining the necessary services and resources. The Transitions of Care Program's goal is to facilitate smoother transitions, bolster member engagement, and provide targeted support to high-risk consumers. Summary of position : This position will focus on member engagement and care coordination for individuals transitioning from inpatient psychiatric care to community-based services. The HLOEP Specialist will meet with individuals while still inpatient and offer THL community-based services to the individual. If the individual accepts THL, he / she will be enrolled and the HLOEP Specialist will provider THL services in an effort to close gaps identified by MCO. TYPICAL WORKING CONDITIONS / ENVIORNMENT This position is a community-based position. The HLOEP Specialist is provided with all equipment necessary to remain mobile and be able to work from the community or in personal vehicles. HLOEP Specialist does have access to office space on-site and meeting space for office visits if clients prefer this method of follow-up and outreach. HLOEP visits with individuals who are currently inpatient at local psychiatric inpatient facilities. HLOEP specialist participates in Treatment Team and Team Meetings as outlined by supervisors upon hire. This job description does not constitute a written or implied contract of employment. JOB DUTIES / RESPONSIBILITIES 1. Works in a multi-disciplinary team approach to meet the clinical needs of SPMI consumers. Begins and ends workday as scheduled and is accessible by phone when working in the field. Attends team meetings and treatment teams as requested by supervisor. Responds to all flags, emails, and voicemails within 1-2 business days. Submits to Services Coordinator, or designee, proposed schedule for the following week by the stated deadline. 2. Completes all documentation in compliance with CARF and funder standards. Obtains individual's signature either accepting or declining THL Services. Submits all Daily Contact Logs and signed documentation within 2 days of the occurrence. Progress Notes are clinical in nature and contain all relevant information. Progress Notes are entered into the Electronic Medical Records within 2 business days of the encounter. 3. Provide linkage and / or referral to agencies / providers for treatment of mental health and / or medical needs. Once individual is enrolled in THL, ensures that the individual has an up to date Provider Communication Log and ROI for their PCP and any other medical providers. Once individual is enrolled in THL, works with team members to ensure client is referred to appropriate programs to meet needs. 4. Monitoring of compliance with quality measures. Ensure that the individual has a follow-up after hospitalization appointment scheduled and that the individual attends appointment within 7 days of discharge. Engage individuals who utilize higher levels of care (i.e. CSU, RTF, and inpatient hospitalization) in THL. Focus on decreasing readmission rates to inpatient hospitals and RTS within 30 days of discharge. 5. Team Atmosphere Interacts professionally with other employees, clients, and community organizations. Works effectively as a team contributor on all assignments. Works independently while understanding the necessity for communicating and coordinating work efforts with other employees and organizations. Is expected to have regular and predictable attendance and the ability to work cooperatively with others. COMPENSATION : Starting salary for this position is approximately $21.50 / hr based on relevant experience and education. Schedule : This position works a flexible schedule given the responsibilities of the position. Staff can expect to work mainly Monday-Friday either 8a-5p. Travel : This position requires utilizing a personal dependable vehicle to conduct Center business. Maintaining a dependable vehicle and certified driver status (obtain F endorsement) is a condition of employment. Staff members will be required to provide transportation in their personal vehicle and staff members are reimbursed for use of their vehicle for this purpose. Equipment / Technical Competency : This position requires utilizing a personal dependable vehicle to conduct Center business. Maintaining a dependable vehicle and certified driver status is a condition of employment. This position does require basic computer skills for timekeeping and use of electronic medical records entry. Use of a mileage application on a Center issued phone is also required for mileage reimbursement. QUALIFICATIONS - Hospital Liaison Outreach & Engagement Specialist Experience / Knowledge : One-year experience in the social work field or a behavioral health setting preferred. Education / License : Must have at least a Bachelor's degree in a health-related field of counseling, psychology, social work, sociology, or a behavioral science field with course work or experience in the areas of cultural diversity, human development, etiology and treatment of mental illness, alcohol and drug abuse, physical and sexual abuse, suicide, and mental disabilities. Candidates with a Bachelor's degree in rehabilitation, occupational therapy, criminal justice, or education, must have fifteen college-level semester hours of coursework in behavioral health and at least one year of work experience in the behavioral health setting. Physical / Emotional / Social - Skills / Abilities : Exposure to biological hazards. Hearing of normal and soft tones. Close eye work. Valid driver's license- "F" endorsement. Lifting up to 50 lbs. Pushing / pulling up to 150 lbs. Frequent sitting, standing, walking, bending, stooping, and reaching. Required to be certified in, and effectively implement, verbal and physical de-escalation techniques that include a wide range of bodily movements; including but not limited to, grasping, holding another person, going down on knees, running, and walking.
