Responsibilities

Evaluate and treat home health patients requiring speech-language therapy.

Perform therapy interventions utilizing standard therapy techniques and skills as appropriate for the condition of the patient.

Educate and counsel patient and family regarding treatment plans.

Maintain clinical records and follow performance improvement recommendations.

Plan and / or assist with patient discharge from home health speech therapy services.

Provide information regarding appropriate support programs.

Requirements

Licensure / Certification / Registration :

BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.

Drivers License credentialed from the Illinois Department of Motor Vehicles (IL-DMV) obtained prior to hire date or job transfer date required.

Speech-Language Pathologist credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date required.

Education :

Bachelor's degree required.

Master's degree preferred.

Additional Preferences

Bachelors in Speech pathology Required.

Minimum of 2 years of geriatrics experience strong preferred.

Home Health experience Strongly Preferred.

Daytime Weekdays- 6 visits a week, no weekends or holidays, flexible schedule.
