Field Engagement Excellence

• Conduct strategic in-person outreach to doctor's offices, hospitals, and patients' homes to educate potential patients and caregivers about our mission and services
• Navigate complex healthcare and social service systems to effectively guide patients through care options
• Schedule and facilitate recommended healthcare appointments, ensuring seamless transitions into ongoing care
• Build and strengthen network of trusted referral partners in the community
