Job Summary
Health Care 4 All, Inc. is looking for an experienced Clinical Manager in Home Care to join our growing team. The Clinical Manager is the person who manages all patient care services including planning, coordination, leading, controlling, and evaluation of home health services. This position requires visit patients at home, a strong understanding of home health, geriatrics, and pediatrics, and excellent organizational and communication skills. To succeed, you should have demonstrated knowledge and skill in current nursing practices and a strong penchant for solving problems.

Responsibilities

• Consults, plans, and works with Administrator, in policy formulation and programmatic development and implementation, including assessing adequate and appropriate staffing.
• Evaluates the program, services, and field personnel.
• Directs the implementation of service goals and objectives.
• Oversees the implementation, and evaluation, of client care services inclusive of regulatory licensure and certification criteria and accreditation standards.
• Ensures the development, implementation, and updates of the individualized plan of care.
• Ability to step in and complete assessments and visits when needed.
• On–Call Requirement
• Ensure follow the Medicare and Medicaid guidelines.

Qualifications

• Currently licensed Registered Nurse (RN) in state of employment
• Must have at least two years of homecare experience and, at least, one year of supervisory experience in a team leader role.
• Strong leadership and organizational skills
• Valid driver’s license
• First aid/CPR certification required
• Languages: Proficient in English and Spanish
