LPN - Evening Shift

Licensed Practical Nurse (LPN)

Job Description:
We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our team in New York, NY. The LPN will play a vital role in delivering quality care to our residents and ensuring their comfort, safety, and well-being. In this role, you will perform clinical functions and provide direct patient care in accordance with established protocols and under the direction of RN Supervisors. Charge nurse responsibilities may be assigned as needed.

Responsibilities:

• 
  Provide nursing care to residents in accordance with their care plans
  
• 
  Administer medications and treatments as prescribed
  
• 
  Monitor patients’conditions and report significant changes to the RN or physician
  
• 
  Maintain accurate medical records and documentation
  
• 
  Assist with assessments and care planning for residents
  
• 
  Supervise and support CNAs and other support staff as needed
  
• 
  Ensure a clean, safe, and comfortable environment for all residents
  

Qualifications:

• 
  Valid LPN license in New York State
  
• 
  Previous experience working with geriatric populations required
  
• 
  Graduate of an approved practical nursing program
  
• 
  Strong clinical and critical-thinking skills
  
• 
  Excellent communication and interpersonal skills
  
• 
  Ability to work independently and as part of a multidisciplinary team
  

Pay Rate: $37.50 Per Hour
