Job Description

Job Title : Home Health Physical Therapist

Job Specialty : Therapy

Job Duration : 13 weeks

Shift : 8 : 00am - 5 : 00pm

Guaranteed Hours : 40 hours per week

Experience : Not specified, but prior work experience as a therapist in a clinical setting is helpful.

License : National registration and licensure for the state of practice, if required.

Certifications : Completion of an accredited physical therapy program required.

Must-Have : Must be at least 18 years of age. Basic physical therapy skills. Ability to effectively communicate in English, both orally and in writing.

Job Description :

  • Assess clients' needs and develop therapy plans that meet identified needs.
  • Administer and perform therapeutic interventions with clients.
  • Maintain clients' self-esteem, privacy, and confidentiality of personal information.
  • Provide clients with opportunities for independence and choice consistent with their care plans.
  • Notify physicians of changes and process orders.
  • Document observations and services provided.
  • Provide direct consultation with physicians and / or interdisciplinary team members; receive and share information.
  • Provide training on programs, adaptive equipment, and other interventions.
  • Utilize and supervise therapy assistants as outlined by Federal and State regulations and document appropriately in the clinical record.
  • Use appropriate bag techniques and standard precautions.
  • Follow infection control and safety standards when using equipment.
  • Comply with all related laws, regulations, company policies, and procedures.
  • Responsibilities may change at any time with or without notice.

  • The noise level in the work environment is usually moderate.
  • The employee must be able to lift up to 50 lbs.
  • The role includes supervising and managing one Physical Therapy Assistant (PTA).
  • Up to five cases per day, focusing on single-family homes, apartments, or assisted living facilities.
  • The software used includes Remote Client / Epic.
  • Training for new graduates is provided.
  • Productivity expectations are at 85-90% once trained.
  • Mileage and drive time are reimbursed.
