$72,592.00 - $107,254.68

Position Overview

Community Based & Life Center

  • Triages and assesses Participants in the home clinic setting and providing education resources.
  • Participate as a member of the interdisciplinary team (IDT), including completion of initial medical history, physical exam, and functional nursing assessments of each new participant and semi-annual, annual, and unscheduled assessments; communicates changes in participant health or functional status to the interdisciplinary team members, and participates in development of the plan of care and coordination for 24-hour care delivery.
  • Participates in daily program activities and provides comprehensive plan of care to meet participant needs and promote optimum health, independence and self-care.
  • Communicates, counsels and guides participants and families regarding service needs and concerns; makes referrals as appropriate.
  • Collaborates with Clinic RNs and RN Case Managers, regarding care delivered to Participants in the clinic and any transitions of care, through verbal and electronic communication.
  • Maintains current written / electronic records and documents related to participant care.
  • Actively serves on the Quality Committee. Involved in the development and implementation of QAPI activities.
  • Conducts comprehensive nursing assessment of participants, at appropriate intervals, at times of status changes Registered Nurse (RN) and as needed with each participant, family members and caregivers.

MINIMUM REQUIREMENTS

  • Education : Associate's degree or graduate from an accredited school of nursing. BSN preferred.
  • Experience : One year working with the frail elderly population.
  • Other Credentials : AHA BLS - Healthcare Provider,Drivers License,Registered Nurse - NJ or Registered Nurse - NLC multi-state
  • CPR Requirements :

  • Requires ACLS (or must obtain within 6 months of hire date) if assigned to : Critical Care / Intermediate / Telemetry, Emergency Rooms, Pediatrics / Pediatrics Emergency Room, Labor & Delivery, Surgical Services (not to include Perioperative), Interventional Procedures, Observation.
  • Requires NRP (or must obtain within 6 months of hire date) if assigned to : Maternity Services, Emergency Room RMC / Deborah
  • Requires PALS (or must obtain within 6 months of hire date) if assigned to : Emergency Rooms, Infant Follow-Up, Surgical Services (only PACU & Same Day Surgery), Pediatrics / Pediatric ED.
  • Knowledge and Skills : Possesses strong problem solving and decision making skills. Demonstrates high interpersonal skills at an individual as well as team level. Excellent verbal and written communication skills. Adjusts quickly and reacts positively to change. Considerable knowledge of principles, practices and current trends in nursing. Possesses good work ethic.
  • Special Training : Experience working with clinical charts and records.
  • Mental, Behavioral and Emotional Abilities : Must have the ability to understand and standardize process related to quality indicators and utilize technology to document and present data.
  • PHYSICAL DEMANDS AND WORK ENVIRONMENT
