A dedicated Internal Medicine Physician is sought to provide primary care for seniors, emphasizing comprehensive assessments and individualized care plans. This role includes collaboration with an Interdisciplinary Team for effective care coordination and oversight in various settings, including nursing homes and hospitals. Qualified candidates should have board certification in Internal Medicine, Family Medicine, or Geriatrics and possess at least 5 years of post-residency experience.

Internal Medicine or Family Medicine Physician

Position Overview :

Seeking a Primary Care Physician to join a comprehensive care program focused on seniors. This role involves in-person assessments, care coordination with an Interdisciplinary Team (IDT), and oversight of participant care in various settings.

Key Responsibilities :

  • Conduct comprehensive assessments and develop individualized care plans.
  • Collaborate with IDT and community physicians for discharge planning and care delivery.
  • Oversee care in nursing homes, hospitals, and participant homes.
  • Provide direction to Nurse Practitioners and participate in rotating night and weekend call.

Qualifications :

  • Board Certification in Internal Medicine, Family Medicine, or Geriatrics (preferred).
  • Active, unrestricted California Physician License, DEA, and CDS certification.
  • Eligible for Medicare and Medi-Cal enrollment.
