LPN Coordinator

Trustbridge Hospice, a part of Empath Health is seeking a caring and motivated LPN Coordinator/Dispatcher to work with us in our West Palm Beach location to provide coordination and dispatch of our staff visits on the following schedule: Monday - Friday 8:00am – 4:30pm.

What you'll Do

• The LPN Coordinator is responsible for coordinating with Managers and team members in patient/family direct and indirect care needs and provides quality professional intervention based on protocols and policy regarding the physical, emotional and psychological needs.
• Interacts with management, physicians and field staff utilizing a teamwork approach.
• Serves as a professional leader in assessing, planning, guiding and evaluating patient care activity.
• Pledge to make the comfort and care of patients and families the highest priority.
• Consistently provide a high level of personalized, sincere and engaging service to patients and families at all times.
• Ensure the complete satisfaction of the patient and family.

What You'll Need

• Current license to practice as a Licensed Practical Nurse in the State of Florida.
• Current Basic Life Support Certification.
• Minimum experience of one year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing or Hospice nursing.
• Understanding of the ability to apply the characteristics of hospice nursing, case management, crisis intervention, communication skills, thanatology skills and needs.
• Basic computer skills and EMR software experience a plus.
