LPN Coordinator

Trustbridge Hospice , a part of Empath Health is seeking a caring and motivated LPN Coordinator to work with us in our West Palm Beach location to provide coordination of our staff visits on the following schedule : every Friday / Saturday / Sunday 8 : 00am – 8 : 30pm.

What you'll Do

• The LPN Coordinator is responsible for coordinating with Managers and team members in patient / family direct and indirect care needs and provides quality professional intervention based on protocols and policy regarding the physical, emotional and psychological needs.
• Interacts with management, physicians and field staff utilizing a teamwork approach.
• Serves as a professional leader in assessing, planning, guiding and evaluating patient care activity.
• Pledge to make the comfort and care of patients and families the highest priority.
• Consistently provide a high level of personalized, sincere and engaging service to patients and families at all times.
• Ensure the complete satisfaction of the patient and family.
