Job Description

We're seeking a dedicated and compassionate Physical Therapist (PT) for a contract position in a Skilled Nursing Facility (SNF) setting. This role is based in Wabasso, Minnesota , with scheduled floating to Franklin, MN two days each week.

This is a great opportunity for a motivated Travel PT or Contract Physical Therapist who enjoys working in a collaborative environment, focusing on quality care and patient progress in a supportive SNF setting.

Responsibilities

Evaluate, plan, and implement physical therapy treatment programs for patients in SNF and sub-acute rehab settings.

Create individualized therapy plans to improve strength, balance, and mobility.

Accurately document treatments, progress, and outcomes in accordance with facility and payer requirements.

Collaborate with OTs, SLPs, nursing, and interdisciplinary team members to deliver patient-centered care.

Maintain an 85% productivity standard.

Provide therapy coverage at both Wabasso and Franklin facilities as scheduled.

Qualifications

Current and valid Physical Therapist (PT) license in the state of Minnesota (or eligibility for MN licensure).

Graduate of an accredited Physical Therapy program.

Previous SNF or geriatric rehab experience preferred.

Excellent communication, documentation, and organizational skills.

Reliable transportation for travel between sites.
