Washington Licensed RN Case Manager

A company is looking for a Bilingual Case Manager, RN - Maternal Health.

Key Responsibilities :

Engage members to complete comprehensive needs assessments including medical, behavioral, and socioeconomic factors

Develop and implement person-centered care plans addressing chronic health conditions and promoting wellness

Collaborate with care teams and community resources to support member self-management and care transitions

Required Qualifications :

Current, unrestricted RN licensure in the state of Washington

2+ years of Maternal clinical experience

1+ year of experience with MS Office applications

Bilingual in English and Spanish

Designated quiet workspace with secure high-speed internet access
