Our home health organization is currently experiencing growth, and we are looking to add a home health Community Liaison to join our team. We serve a large portion of the Chicagoland area. This opportunity is a full-time position but has flexibility with the schedule. We are located in the Schaumburg area.

The Community Liaison develops and maintains business relationships for home health community based referral sources and providers. This marketing representative acts as a resource for information to ensure quality care for every patient and family that we serve. This position will primarily be on the road visiting facilities, hospitals and doctor offices. We would like the marketer to come in 1 to 2 times a week to go over progress and strategy.

Responsibilities :

• Partner with management to meet admission targets
• Assist in the verification of patient referral service authorizations
• Address client satisfaction issues
• Conduct daily in-person visits and activities to meet / exceed the census goals.
• Prospect territory to expand referral base.
• Evaluate competition for opportunities to improve marketing program. Cultivate and maintain relationships with referral sources that include physicians and their staff, hospital discharge planners, and social workers.

Qualifications :

Prior Healthcare Sales experience required

Strong communication, organizational, and time management skills

Highly motivated towards achievement
