About Us
Senior PsychCare provides collaborative behavioral health services to residents in long-term care facilities. Our multidisciplinary teams include Board-Certified Psychiatrists, Psychiatric Nurse Practitioners, Physician Assistants, Licensed Psychologists, and Licensed Clinical Social Workers / Licensed Professional Counselors / Licensed Marriage and Family Therapists. We focus on continuity of carenot triageensuring our patients receive comprehensive and compassionate services to achieve their best quality of life.
What Youll Do
- Conduct cognitive and emotional assessments to set / manage treatment plans
- Manage psychotropic medication and gradual dose reductions
- Collaborate with nursing home staff and SPCs behavioral health team
- Participate in treatment team meetings (in-person and virtual)
- Deliver care both in-person and via telehealth when appropriate
- Maintain accurate, timely documentation in our EHR system
