As a Occupational Therapist, you will :
Provide Direct Care – Evaluate clients by obtaining and interpreting subjective and objective data. Develop personalized treatment plans with short-term and long-term goals, ensuring they align with physician orders. Maintain high standards of care while adapting to different environments and client needs.Maintain Accurate Client Records and Billing – Document all services accurately and timely using Vivie’s EMR system. Demonstrate a thorough understanding of all payer sources and authorization requirements. Document collaboration with COTAs and perform supervisory visits for HHAs as per regulatory guidelines.Develop and Support Follow-Up Programs – Recommend and oversee continuation programs to support long-term client success and optimize discharge outcomes. Make referrals to continuation programs as necessary, ensuring clients receive the appropriate follow-up.Communicate Effectively and Collaboratively – Maintain clear and compassionate communication with clients, families, referral sources, physicians, and the interdisciplinary team. Utilize exceptional verbal and written communication skills to ensure cohesive and effective care delivery.Ensure Regulatory Compliance – Complete and document supervisory visits with OTAs and RA / CNAs in accordance with state and regulatory guidelines.Other Duties as Assigned – The duties and responsibilities listed above are representative of the nature and level of work assigned and are not necessarily all inclusive.This job also requires :
Current Occupational Therapist (OT) license in the state of Minnesota.CPR certification within 6 months of hire.Valid driver’s license and access to an insured personal vehicle.Willingness to float across regions as needed.Ability to pass state mandated background check.Strong clinical skills in occupational rehabilitation, skilled therapy services, knowledge of evidence-based occupational therapy practices.Proficiency in reading, writing, and speaking English to communicate effectively with clients, families, and team members.Physical capability to perform all job duties.Experience in home health, long-term care, or rehabilitation settings preferred.Additional Details :
