As a NP at Horizon, you will…

• Provide initial psychiatric examinations for individuals admitted to mental health, rehabilitation and recovery programs and provide follow up medication management services.
• Participate in clinical case conference and case reviews to programs to provide consultation and guidance with difficult cases, assures communication in multiple forums with counseling staff to be updated on consumer progress and outstanding issues or concerns.
• Prescribe psychotropic and / or addiction medication to ameliorate symptoms; reviews incoming physical exams to ensure if the information is current and accurate.
• Communicate to primary counselor the need to make referrals for physical exams with consumer's primary care physician.
• Ensure that all confidential and potentially sensitive information is processed, maintained, and utilized in a strictly confidential manner.
• Review and sign all treatment plans for patients enrolled in program as needed.
• Document all services provided in the case record. Must be literate with electronic health records.

Location :

This position would be located at 100 Niagara St, Tonawanda NY. This position requires flexibility to travel to other clinics as needed.

Hours :

This is a part-time position from Monday - Friday with flexible hours.

Compensation :

Horizon is consistently evaluating our compensation and benefit packages to ensure we remain competitive. The range of pay for this position is $65.00 - $80.00 per hour. Starting pay may be impacted based on experience and education including DNP. This position includes a bonus earning opportunity, with an annual max potential of $2,400, based on volume of service visits provided and cash equivalent opt out benefits. Compensation adjustments are evaluated annually.
