Affirmed Home Care is seeking dedicated and compassionate Licensed Practical Nurses (LPNs) to deliver exceptional in-home healthcare services throughout Northern and Central New Jersey .

Key Responsibilities

Provide skilled nursing care under the supervision of an RN and in accordance with care plans

Administer medications, treatments, and wound care as prescribed

Monitor and document patients’ conditions, reporting changes promptly to the RN or supervisor

Assist with IV therapy, catheter care, and other skilled nursing tasks as permitted by scope

Educate clients and families on medication adherence and home safety practices

Maintain accurate, timely, and compliant documentation in EHR systems

Promote a safe, respectful, and supportive environment for clients and families

Qualifications

Active LPN License in the State of New Jersey (required)

1–2 years of clinical experience preferred (home care, long-term care, or related field)

Excellent communication, assessment, and time-management skills

Compassionate, dependable, and able to work independently in a home care setting

CPR / BLS certification (required)

Valid driver’s license and reliable transportation

Experience with Electronic Health Records (EHR) preferred
