• Provide care within the training limitations of a CNA for both home health and hospice patients in their place of residence, including all covered aide services under the Medicare home health and hospice benefits.
• Assist patients with daily living skills to meet the patient’s needs in accordance with assessments, care plans and as directed by nursing management.
• Work under the supervision of a Registered Nurse.
• Interact well with patient, family, physicians, Interdisciplinary Team members and other healthcare professionals.
• Observe patients, reporting and documenting observations and care performed.
• Complete all documentation and charting accurately and in a timely manner
• Demonstrate appropriate and safe knowledge/use of equipment (wheelchair, hospital bed, side rails, wheelchairs, oxygen, etc.) within training limitation.

• Active Utah CNA certificate
• Current CPR certificate
• Valid Utah driver license with car insurance
