Job Summary : Brockton Home Health care Agency iscurrently looking for a full time Home Care Office LPN, also known as a Licensed Practical Nurse (LPN).The LPN will be providing administrative and clinical support, rather thandirect patient care in the home. This role involves tasks like coordinatingcare plans, managing paperwork, review (PA) Prior Authorization beforesubmission to insurance providers, review admission packets for accuracy, andcommunicating with patients and families.
Required Skills :
- Licensed Practical Nurse (LPN) license in Massachusetts.
- Strong knowledge of medical terminology and healthcarepractices.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Experience in a home health setting or office setting ispreferred
- Strong communication skills : Both verbal and written, toeffectively communicate with insurance providers, clients and caregivers.
- Time management skills : To effectively handle multipletasks and meet deadlines.
- Computer proficiency : Internet skills, including MicrosoftOffice suite and Axxess experience
AdministrativeTasks :
Managing referrals, auditingpaperwork, and answering questions.Care Planning : Collaborating withRNs to develop and modify patient care plans, ensuring they arepatient-specific and meet individual needs.Communicating with patients,families, and other healthcare professionals about care plans and patientstatus.Maintaining accurate andup-to-date patient records and documenting care provided.Supervising and delegating tasksto Home Health Aides (HHAsAssisting with medicationadministration, wound care, and other basic nursing tasks, as needed.Current and Valid LPN License :Must be licensed as a practicalnurse in Massachusetts by the Massachusetts Board of Registration in Nursing.CPR Certification :Knowledge of nursing principles,practices, and procedures, as well as experience in home health or a similarsetting is often preferred.Excellent communication,interpersonal, and customer service skills are essential.Familiarity with computer systemsand electronic health records is often required.Benefits :
