Job Description

Job Title : Registered Nurse

Guaranteed Hours : Not Guaranteed

Experience : Minimum 1 Year of Adult Home Health or Hospice Experience

License : Must have License in Hand

Certifications : CPR, BLS

Must-Have :

  • Proficiency in HomeCare HomeBase (HCHB)
  • Experience with OASIS
  • Reliable transportation, a current driver's license, and agency-required liability insurance
  • Comfort with traveling up to 75 miles from the home clinic

Job Description :

  • Provide, coordinate, and direct home nursing care according to physician's orders based on agency policies and procedures.
  • Supervise and evaluate Home Health Aides and Licensed Practical Nurses / Licensed Vocational Nurses.
  • Average 5-8 patients per day, requiring travel from patient home to patient home (mileage reimbursed).
  • Types of cases include Start of Care, Wound Care, Follow-up Visits, etc.
  • Complete all documentation within 24 hours of patient appointments.
  • Attend periodic office meetings.
  • Possibility to float to nearby clinics if patient census is low.
  • Use of Point Care EMR system.

    • Additional Information :

  • Must be proficient in HCHB and OASIS and have used them within the last 6 months, or must complete a retraining course.
  • Local candidates accepted at the same rate.
  • Must be open to on-call rotations based on location and profession.
